The Hospice Team
Attending Physician - Certifies the patient has a terminal illness and remains the patient's primary physician and works as a member of the hospice team
Medical Director - Certifies the attending physician's prognosis. Leads the hospice team in developing a plan of care. Provides consultation to the physician.
Skilled Nurses RN's & LVN's - Evaluates the patient's condition during regular scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques, the use of medication, and pain management.
Social Worker - Assesses social needs of the patient and family, and provides psychological and bereavement counseling.
Chaplain - Provides spiritual support. Works as a liaison with clergy of the patient's choice.
