Our Services
Initial Palliative Assessment
Initial Palliative Assessment conducted by an NP or MD in the patients home including head to toe exam, collection of personal medical history, medication reconciliation, environmental safety risk assessment, depression screening and patient education regarding pain and disease management.
Advanced Care Planning
Ongoing and documented discussions with the patient, family, or other legally recognized decision-maker. Counseling addresses advance directives and POLST forms and help in family conflict resolution over issues surrounding the patient’s decisions
Plan of care
High Care will develop a plan of care individualized to the patient’s needs and goals. The Plan of Care will address the medical, behavioral, and environmental components of a patient’s health. Including pain and symptom management, chronic disease management, medications, and patient specific interventions and goals
In-Home Nursing Visits
Patients will receive scheduled in-home nursing visits while on Palliative Care. Visits will include checking vital signs, disease specific nursing interventions, documentation of progress toward Plan of Care goals, symptom management and patient education.
Case Management & Coordination
Patients have a dedicated Nurse Case Manager to help navigate their disease process and coordinate appropriate services. Nurse Case Manager ensures the continuous assessment of patient's needs and adherence to plan of care.
Medical Social Services
Patients will receive a psychosocial assessment completed by a Medical Social Worker following the Initial Palliative Assessment. Social Worker will help connect patients with appropriate behavioral and community resources, assess environmental safety, assist with caregiver support services applications, and support in advanced care planning
Spiritual Care Services
Patients will receive spiritual care by a Spiritual Care Counselor following the Initial Palliative Assessment. Our Spiritual Care Counselor discusses spiritual, religious and existential aspects of coping with the disease process.
24/7 Access to Nurse Triage
Patients have access to the Palliative nurse triage line 24 hours a day, 7 days week. Nurse triage can assess patients telephonically, send nurses for in-home visits, and/or consult an on-call physician
